Jeferson sent a message on his phone at 10am before the device lost coverage.
His sister Sheila Gomes da Silva reported his disappearance to the police.
She said he had no physical health problems and that he had never gone missing before.
The family went to all the health facilities in the area, but no one had seen Gomes – who is autistic.
A CCTV camera had caught him running along a trail in the park in Santo André, on the outskirts of São Paulo, Brazil, the day he went missing.
After searching the area, local police found him down a well on Wednesday morning, much to his desperate family’s relief.
The shaft is located in a wooded area of the park and is 52 ft (16m) deep, according to local media.
Jeferson, who was conscious, told the officers he had fallen down the hole on Saturday.
He was winched to ground level yesterday (5 Jun).
He had been given a hard hat and goggles before he was pulled up the deep shaft.
The footage shows him covered in dirt and dust as his rescuers lay him on a stretcher.
When they asked him about his condition, the teen said he felt pain in his ankle and knee and was very thirsty.
Firefighters were called in to carry out the rescue, as reported on Need To Know, and they abseiled down the hole to reach him.
After being winched to the surface, Gomes was treated by paramedics.
At least 10 people were involved in the rescue, according to reports.
The authorities say he was taken to a local hospital with a fractured leg.
