Eyewitnesses said the wheel had completed two rotations when loud creaking noises began.
Despite warnings from onlookers, the operator allegedly kept the ride running.
Moments later, the 60ft structure gave way, trapping several people beneath it.
Up to 80 people were on board at the time, with at least 30 – including women and children – injured in the incident on Wednesday (1 Apr) night.
Emergency services and bystanders rushed to help those hurt at the Bhainsaha Mela fair in Khadda, Kushinagar district, Uttar Pradesh state, India.
It reportedly took nearly an hour to free everyone trapped in the wreckage, as reported by Need To Know.
Local reports say ambulances transported at least 10 critically injured people to a nearby hospital.
Two children are said to be in serious condition.
Preliminary findings suggest the base of the ride was weak, poorly anchored, and unable to withstand the “excessive” load.
Authorities are probing possible negligence and safety lapses.
